Complete Each Semester - New and Returning Students

The Family Educational Rights and Privacy Act (FERPA) of 1974 is a Federal law that protects the privacy of student educational records. This release form allows you to provide access to your financial records for the purpose of assisting you with the financing of your education, your academic information and/or your housing information.  You can change who has access to this information at any time. This form is electronic and will require logging in to the student portal to complete.

The student financial agreement is required each semester. It ensures you understand your commitments and expectations as a Beacon College student.

In accordance with the Higher Education Opportunity Act of 2008, Beacon College has a procedure to investigate when a student is reported missing. Each student has the option to designate a confidential contact by submitting this form to Housing & Residential Life. This person will be contacted within 24 hours of the time that a student is considered missing. Law enforcement officials (including Campus Safety) also will be notified of a student’s absence no later than 24 hours after the student has been deemed missing, and will follow their own investigation procedures. However, keeping your confidential contact up-to-date about your whereabouts will help the investigation considerably.

The Student Health Insurance “Wellfleet”  Opt-Out waiver is available after Jul. 1, 2023. You must complete the form until you receive a confirmation code to waive the student health insurance plan. Failure to waive the health insurance will result in in the annual fee being applied to your fall 2023 billing statement. It is critical that we have the most up-to-date information for current students' address and phone number. Please complete this form each semester to ensure that you receive appropriate communication from Beacon College.

The Beacon College Housing contract must be completed each semester that you intend to live on campus. This contract outlines the agreements and expectations for students living in on-campus housing. 

Community Health Centers, equipped with a registered nurse designated for Beacon students, offers a wide range of private and confidential services to ensure that our students are physically able to meet the challenges of academic life. Grant or refuse Beacon College with the right to use photographic or video likenesses of you in educational, directory, marketing or other lawful purposes.

This waiver is required for any student who wishes to use Beacon's on-site recreational facilities. (For students under 18, please request the parent/guardian version of this waiver.)

Complete Once - New Students Only

Beginning April 10th, students will begin to receive the New Student Housing Application via email upon receipt of their $525 deposit & intent to enroll form.

Beacon College must get your correct identifying number to file Form 1098-T, Tuition Statement, with the IRS and to furnish a statement to you. This will be your Social Security Number (SSN) or, if you are not eligible to obtain an SSN, your individual taxpayer identification number (ITIN). Form 1098-T contains information about qualified tuition and related expenses to help determine whether you, or the person who can claim you as a dependent, may take either the tuition and fees deduction or claim an education credit to reduce federal income tax. This form concerns your medical immunization history and must be completed by a physician. Completion of this form is necessary to comply with the Florida Statute 1006.69 and the Florida Admin Rule 6C-6.00l(S). 

This required form must be completed by a physician prior to your arrival on campus. 

This required medical history form can be completed by the student and parents/guardians.

This form is only necessary IF the student has not received and declines to receive a meningitis vaccine.

This form grants permission to the Beacon College Student Health Services staff to render any first aid/health care or emergency treatment to the student. It also grants permission for these staff members to arrange health care, emergency treatment or hospitalization at the accredited hospital or psychological facility when deemed medically necessary, and release confidential information to these providers when required to continue your medical care.

This athlete medical history form should be completed jointly by you and your physician. 

This form is a supplement to the athlete medical history form, and is only required for incoming student athletes who also have a disability.
